IF YOU ARE A PARENT WHOSE SERVICE HOURS ARE COMPLETED AFTER REGULAR SCHOOL HOURS, please print the following form (and as many copies as you need) to keep an accurate record of your volunteer service to our school. We will request a copy of them around the month of February, for various school awards.
